� Yes, I have read the terms of participation and understand that:
o I am responsible for paying for my 2 way airline ticket and for Insurance
to cover my stay in Lebanon
o I am responsible to pay $500 as my portion of the expenses during my stay
in Lebanon. The WLCU world Organization will pay the rest
o During the time of the visit, I must follow the schedule of the trip
planned by the organizers.
o Any additional time that extends the period as defined for this visit
(Aug 16 – Aug 26) of this visit will be on my own responsibility.
� I am between the age of 18 (complete) and 35 years.
� I am of Lebanese descent and I hold a valid US Passport.
� I would like to be considered for the $ 500 sponsorship by the WLCU Boston Chapter to cover my part of expenses for my stay in Lebanon. The 5 winners will be selected by lottery and the money will be transferred directly to the World organization.
